GPCCMP in Practice: Care Plans, MyMedicare and Allied Health Coordination
Follow one patient, Mara, from “can I have five physio visits?” to a plan, a referral and a review. Learn the item map, the three clocks, and what to say when a patient asks “am I covered?”.
Already completed this training? Quick refresher
A quick reminder for experienced staff. Revisit only what you need.
- Check history and MyMedicare position before promising the plan pathway.
- Keep plan, review, referral validity and annual allied-health allowances on separate clocks.
- A sent message or paid claim does not prove that care or service requirements are complete.
Open the lessons and tools. Use current local procedures and sources.
This optional refresher does not record a new completion. Certificates require the full course and its assessments.
For you if…
Reception, the care-plan nurse, and the practice manager who runs the pathway and the legacy transition list. Clinical decisions stay with the GP. Each lesson is marked for reception, nurses or practice managers.
What you’ll practise
- Say who does what, using the one-page item map.
- Check MyMedicare status, plan history and the visit count in the right place.
- Keep plan currency, referral validity and the calendar-year count apart, and explain them to a patient in plain words.
- Book face to face or video (there are no phone care-plan items).
- Chase a referral that never arrived, close an outage properly, and work a legacy GPMP/TCA list down before 30 June 2027.
How it works
Nine short, case-based lessons, printable tools and two automatically scored quizzes. Try each decision before opening the worked example. Allow about 50 minutes, including the assessments.
Pass the knowledge quiz with at least 80% and the safety quiz with 100%. Read the feedback and retry as needed. The final case is practice only.
The people and practice in this course are made up. The plan extracts in the lessons show the shape of a plan; they are not complete clinical plans that would support a claim. This course is professional development, not accredited CPD.
