75+ Health Assessment: A Practical Course for Nurses
Run a person-centred 75+ assessment from invitation to follow-up plan: who is eligible, how to book it, what it must cover, what to keep when time runs out, and how to hand over so the GP can act.
AUD $10 including GST · One-off enrolment · 12-month access · Self-paced
Already completed this training? Quick refresher
A quick reminder for experienced staff. Revisit only what you need.
- Check history and the actual interval before booking a Medicare-funded assessment.
- Agree nurse and GP roles, eligible time and safe arrangements for any home visit.
- Prioritise urgent concerns, agree useful actions and close the follow-up and report-delivery loop.
Open the lessons and tools. Use current local procedures and sources.
This optional refresher does not record a new completion. Certificates require the full course and its assessments.
Who this is for
Practice nurses supporting GP-led health assessments in Australian general practice; lesson 1 also suits the receptionist or practice manager who builds the invitation list. Billing detail sits in a collapsed box for billing staff.
The people and practice in this course are made up.
Learning time: allow about 45 minutes, including practice and assessment. Take the time you need.
What you will practise
- Checking age, care setting and the 12-month interval, including an assessment claimed elsewhere, and the order with a heart health check.
- Inviting patients safely: what the data tool can’t see, whose email it is, the words to use, one contact register.
- Booking nurse time and GP time together, a safe home visit, the practice’s emergency code.
- Covering every required area, describing real function, and what to keep when time is short.
- Recognising the fall, the confusion and the medicines discrepancy that need the GP now.
- Including carers on the patient’s terms, offering the written report, uploading a shared health summary with consent.
- Handing over in a few lines and telling a complete service from a completed template.
Eight practical lessons
- Eligibility: twelve months is not a calendar-year reset
- Plan roles, time and a safe home visit
- A complete assessment starts with the person
- Falls: understand the event and the person’s goal
- Cognition and mood: assess change, avoid labels
- Medicines: reconcile what is actually taken
- Carers, consent and a plan that can happen
- Complete the clinical work, then check the claim
Learn by making decisions
Realistic cases where you choose a response and see feedback, a booking model, a “When time is short” box, before-and-after handovers and recall checks. A six-page workbook holds two practice tools with worked examples; the tools are also in the final lesson as text.
Assessment and completion
Complete all eight lessons, pass the 7-question knowledge assessment (80%) and the five-question safety assessment (100%). There is no time limit; you can read the explanations and retry. Practice tasks are not marked. You receive a certificate of completion. This is professional development, not accredited CPD, and gives no clinical or claiming authority.
Use Mark Complete when you finish a lesson, then return through My Courses.
